Who is Eligible?
Your patient may be eligible for Home Health Services if your patient is determined to be homebound. This means that leaving home takes considerable effort or their condition keeps them from leaving home. This can be permanent or temporary and may include some of the following conditions:
- Have a new condition or exacerbation of an existing condition.
- Have recently been hospitalized.
- Have a change in medication or a new medication that requires management/education.
- Require therapy services or are at risk for a fall.
How we communicate
We understand that communication and collaboration are important parts of ensuring the best possible care for your patient. We will communicate with you in your most preferred manner whether that be direct contact with our healthcare professional or by fax.
- Contact after initial visit for discussing plan of care
- Significant change call immediately
- Every 60 days send summary of care provided and outcome of goals
- Send discharge summary
- Send patient specific outcomes
